Cancellations -
New Patients
- 48 hour cancellation required
Existing Patients - 24 hour
cancellation is respectfully requested - barring accidents, sudden illness - first time a $5.00 fee will incur
after that payment in full for the missed session will be required
No Call - No Show
- patients who fail to call or not show up for scheduled appointments are required to pay for missed
session(s) in full, in order to continue with treatment
Timeliness - It
is crucial that patients arrive on time for their appointment. If a patient is late the
session will begin at their time of arrival and end at the alotted time. Full payment is required.
(Please note that as a practitioner, I take
my commitment to the patient very seriously. I request that
the
patient honors their commitment as well. Appointments are scheduled in a manner that is integral
to the patient's healing process.)
Payment - due at the time of receipt
of treatment - no exceptions
Cash
or check is accepted payment
Third party insurance
is not accepted. Checking with one's insurance company is advised.
Some
carriers do cover acupuncture. As well as, some health savings accounts re-imburse for acupuncture.
Those patients pay out of pocket, a statement of treatment is provided, it is submitted by the patient
to their insurance company
and the patient is reimbursed.
